Introduction

The Pharmacy is a science, practice, or profession of preparing, preserving, compounding, and dispensing medical drugs and it is a place where medicines are compounded or dispensed under the purview of Drugs & Cosmetic Act 1940 and the Pharmacy Act 1948. The Pharmacy practice not only includes traditional roles such as compounding and dispensing medications but also includes more modern services related to health care, including clinical services, reviewing medications for safety and efficacy, and providing drug information & patient Counseling.

  • The Department of Pharmacy, Lok Nayak Hospital is situated at Room No. 20 ,New OPD Block , Ground Floor.
  • The Department of Pharmacy renders Pharmacy Services to the patients over the prescription of various department of OPD , Casualty, IPD discharge etc.
  • Out of 49 sanctioned posts( as per the assessment of AR department ) of Pharmacist for counters, 25 pharmacists are rendering pharmacy services to around 8000-9000 Patients per day.

PRESCRIPTION & DRUG DISPENSING INSTRUCTIONS

All Doctors should prescribe drugs by ‘GENERIC NAMES’ and written in legible hand writing/ capital letters and sign with proper stamp.

Prescribed Medicines shall be issued only when the OPD card is registered and stamped on same date from registration counter.

All prescriptions must clearly indicate medicine Name, Dosage and Duration each time it is prescribe. The Pharmacy will not accept prescription written as ‘CST’ on new & additional OPD Cards

Special Drug Forms (SDFs):

  1. Only special medicines which need SDF should be written on SDFs, instead of all prescribed medicines.
  2. Prescription on SDF must be identical to that written as on the OPD Cards.
  3. SDFs are to be signed and stamped by SR OR above / Faculty of the concerned Clinic/Department or an Authorized Signatory for this purpose which must be duly notified by the DMS (OPD) in writing.

Multiple OPD Cards by single patient will not be honored.
Dispensing to minor shall be done only when accompanied by a guardian.
Issued drugs will be marked as tick (v) and not available ones as cross (x) or N.A. in the OPD card .
Pharmacy will dispense the medicines on the same day during working hours.

Drug duration dispensing schedule mentioned below:

Duration Remarks
SOS Specify the number of Tablets / capsules.
Upto 3 days For Casualty/ A&E department.
Upto 7 days All antibiotics & Discharged patients from the wards.
1-14 All the patients’ morning OPD/Special Afternoon Clinics (excluding antibiotic).
1-28 Hematology/ Nephrology/Gyane-ANC & hormones (As prescribed up to 28 days) / Inhalers / anti-Epilepsy medicines
Single unit per prescription Syrup, ear drop, ointment, nasal drop, gargle , mouthwash, cream, lotion etc.

Saturday 9:00 AM to 1:00 PM
Sunday 9:00 AM to 1:00 PM
