OPD Registration Steps
- Token issued to patients by Guard in front of Main OPD registration gate.
- There are separate counter windows (Ground Floor Main OPD Block) for patients as follows:-
Counter 1, 2 | New patient registration (Men) |
Counter 3 | Peds (0-12) New & Old |
Counter 4 (a) | TB Center, Hapatology (old & New) |
Counter 4(b) | Specially abled patient |
Counter 5 | Senior Citizens (New & Old) |
Counter 6,7,8 | New patient registration (Men) |
Counter 9,10,11,12,13 | New patient registration (Women) |
Counter 14 | Lok Nayak Hospital Staff & Govt Employees |
- Separate registration counter in Orthopedics block, for orthopaedics patients
- Patients reporting to Emergency have a different counter in front of Emergency Department (Gate No. 2) and Female Emergency (Gate No. 2)
- Id proof is requested from all patients being registered. However, it is not mandatory.
- If ID Proof is not available, the Data Entry operator asks patient for personal details like Name, Spouse Name, Gender, Age, Address, Mobile No., department. This data is entered in Hospital HIMS (Hospital Information Management System).
- Registration card is printed and handed over to patient. The card has patient details and Department details, such as Unit head’s name and room number.
